Please be advised that in some instances, treatment plans may chance mid procedure. You will be notified of any changes the day of the procedure. Additional charges may apply. While we make every effort to anticipate changes, treatment plans and appointment scheduling may need to be modified to achieve the best outcome.
Due to the rising cost of credit card processing fees, all credit card transactions will be have an additional 2% administrative fee. A minimum purchase of $20.00 is required for all transactions. Smaller transactions will need to be paid via cash or check.
These discounts will only apply to treatments when paid in full by cash or personal check.
A $35.00 service charge will be applied for all returned checks.
As a courtesy, we will file your dental claim with your insurance company. Your deductible and co-pay, or any portion not covered by your insurance company, is due at the time of service. For patients without insurance coverage, payment in full is expected on the day of treatment. Payment is expected when services are rendered unless prior arrangements are made.
24 Hour Notice Requirement: Appointment changes require at least 24 hours' notice. A $45 charge will apply for missed or broken appointments without proper notice. Patients who fail to show up for an appointment or cancel without adequate notice more than once, may be subject to additional costs or dismissal from the practice. If you need to reschedule, we kindly request advanced notice to avoid disruptions.
We allow a 10-minute grace period for late arrivals. If you arrive more than 10 minutes late, you may be asked to reschedule your appointment, as late arrivals impacts other patients and our ability to provide timely care. We strive to provide high-quality care while respecting the schedule of all patients.
While we don't offer in-house payment plans, we do accept third party financing through care credit. They offer low interest and no interest options. You can easily apply online or call and know instantly if you pre-qualify. A minimum of $200 is necessary to utilize Care Credit Payment.
